Description

To improve practitioner skills in advance care planning (ACP) which will facilitate the execution of transportable physician orders for a subset of patients and families living with advanced illness. Funding will be used to host six onsite Transportable Physician Orders for Patient Preferences (TPOPP) training sessions in Kansas to expand the expertise of interdisciplinary team members who comprise community-based coalitions.
