While significant health improvements have been achieved through medicine and public health, individuals’ health and wellbeing are also influenced by where and how they live, and the unfortunate reality is that tremendous differences in health outcomes still remain for many underserved groups and communities. Health disparities adversely affect groups of people who have systematically experienced greater obstacles to health based on their racial or ethnic group; language spoken; immigration/documentation status; religion; socioeconomic status; age; mental health; cognitive, sensory, or physical disability; sexual orientation or gender identity; geographic location; or other characteristics historically linked to discrimination or exclusion.
On this page, you will find information regarding the Healthy Community Initiative: Improving Health Equity in Kansas Request for Proposal (RFP), which seeks to support leadership teams in cities, towns, counties, or regions in Kansas to engage in advocacy efforts that will improve the health of people in Kansas experiencing poor health outcomes and create a network of community health advocates willing to learn and discover how to effectively impact the health of Kansans using a health equity lens.

Proposals are due by 5 p.m. on Tuesday, April 11, 2017, and must be submitted using the online proposal form provided at the bottom of this page. The following templates will need to be completed and attached to the online proposal:
To confirm your ability to accept a grant from KHF, please review the Grant Agreement template, sign and submit the Acknowledgement of Agreement Review with the online proposal. (Do not modify and/or submit the agreement template with your proposal.)
To help with proposal submissions, KHF will regularly update and answer a list of questions posed at applicant meetings or two KHF personnel.
